Push too hard and you burn out; play it safe and progress slows. Managing stamina, cooldowns, and optimization is the first step toward survival and, eventually, dominance.

Private clinics operate across the city, each with its own risks and limitations. Timing, performance, and consistency determine outcomes, while upgrades and routines let you push further—at least for a while. The results begin appearing throughout the city in unexpected ways, shaping districts, NPC behavior, and daily life.

As your influence spreads, resistance grows. Rivals, authorities, and competing power structures react dynamically to your expansion. Streets become tense, pursuits frequent, and mistakes are punished. Movement through the city remains active and dangerous; escaping trouble is sometimes smarter than facing it head‑on.

Unchecked growth leads to instability. Chaos events reshape districts, alter behavior, and permanently alter how the city feels. Fear spreads quickly, but so does celebration—the city reacts to outcomes, not methods. Your actions push the city toward two extremes: Love, bringing public support and leadership, and Fear, delivering underground control and intimidation. Neither path is inherently right or wrong, and each unlocks different systems, challenges, and endings. Ignore balance, and the city may collapse entirely.
